At-Home Care and Support
Ability Connection’s Texas Home Living (TxHmL) is a Medicaid Waiver program that provides help and support in their own homes to individuals of all ages who have an intellectual disability or related condition. It is overseen by the Texas Department of Health & Human Services.
Our TxHmL team works with each individual to ensure they receive community living, professional and technical support so they can continue to live with their families or independently.
Services Include:
- Day habilitation
- Respite services
- Employment services
- Nursing services
- Dental services
- Behavioral support
- Community support (transportation)
- Occupational therapy
- Physical therapy
- Speech therapy
- Audiology services
- Dietary services
- Minor home modifications
- Adaptive aid
Our TxHmL providers can also provide personal assistance services, habilitation or emergency-response services through the Community First Choice (CFC) program. Individuals can also receive unlimited prescriptions though the Medicaid Vendor Drug Program.
To be eligible for the TxHmL program

- Individuals must have a diagnosis of an intellectual disability or related condition and qualify for Medicaid.
