CLASS Case Manager

The CLASS Case Manager (CM) is responsible for supervising the delivery of each consumer’s Individual Plan of Care (IPC), oversight of the delivery of training and support services, integrating the various aspects of services, recording each consumer’s progress and reviewing the Individual Program Plan (IPP) quarterly to assess progress and continued appropriateness of the program services. The CLASS CM is a member of the person’s Service Planning Team (SPT). The CLASS CM is accountable to the consumer for coordinating his/her overall program services and is administratively responsible to the Program Manager. The CLASS CM is responsible for assuring that all the coordination of services are implemented consistent with the CLASS Evidentiary Principles. The CLASS CM demonstrates leadership and competency through facilitation using person-centered principles and promotion of consumer outcomes. The CLASS CM ensures that services are initiated and delivered in accordance with the IPC and contacts the consumer and/or family at least monthly to verify services. Responsible for documenting all contacts with the consumer, families and service providers in progress notes. Informs the consumer and/or his/her family about the consumer’s overall program and significant changes in it and is available to answer questions about the program asked by the consumer and/or his/her family. The CLASS CM will need to have a flexible schedule and be able to respond in person, if necessary, during consumer crises. Professional behaviors will be expected and evaluations will occur during a normal evaluation period.

Requirements :
(1) a bachelor’s degree in a health and human services field, and two years’ work experience in the delivery of services and supports to persons with related conditions or similar disabilities; or
one of the following:
(i) a high school diploma and four years’ work experience in the delivery of services and supports to persons with related conditions or similar disabilities; or
(ii) a high school equivalency certificate issued in accordance with the law of the issuing state and four years’ work experience in the delivery of services and supports to persons with related conditions or similar disabilities;
(2) is an employee of the CMA;
(3) is not employed by or contracting with a DSA to provide a direct service to an individual served by the CMA; and
(4) is not a relative of the individual to whom the case manager is providing case management.
